Key Responsibilities
The Primary Care Physician will provide proactive, preventive care to seniors, including wellness visits, physical exams, and chronic condition management. They will work within an integrated care team to deliver personalized, compassionate care that improves patient health outcomes.
Requirements
Candidates must hold a Doctor of Medicine or Osteopathic Medicine degree and be board certified or eligible in Geriatric, Family, or Internal Medicine. A valid, unrestricted Florida medical license and at least two years of experience in senior value-based care are required.
