Family Nurse Practitioner

Partnership Health is opening a brand new office in Holly Hill, FL. This office will only be open to the Sherrif's and their families for Volusia County.

We are seeking a Full time as well as Part Time FNP/PA for this new office opening in March of 2027

Full time Schedule - 3 days per week 8am-6pm plus every other weekend Saturday and Sunday from 8am-1pm

Part Time Scheule - 2 days a week 8am-6pm plus every other weekend Saturday and Sunday from 8am-1pm.

Our offices offer onsite Family Medicine, X-ray, Physical Therapy, Pharmacy, Chiropractic, and Behavioral Health.

Minor suturing is required but can be taught if needed.

Minimum Qualifications

• Current and valid Family Nurse Practitioner (FNP) or Physician Assistant (PA) certification and licensure in the state of Florida.
• Basic Life Support (BLS)

Preferred Qualifications

• Previous work experience in outpatient office - family practice, specialty office, urgent care or critical care settings.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
• - - Responsibilities:

• Conduct comprehensive health assessments and physical examinations for patients 2 years and older.
• Diagnose and manage acute and chronic medical conditions.
• Develop, implement, and monitor individualized treatment plans in collaboration with interdisciplinary teams.
• Provide patient education on disease prevention, medication adherence, pain management, and lifestyle modifications.
• Ensure patient safety by adhering to clinical protocols and maintaining accurate medical records.
• Coordinate referrals to specialists and follow up on patient progress and treatment efficacy.

• Full Time 3 days per week plus every other weekend
• Part Time - 2 days per week and every other weekend
• Full time 35 hours
• Part time 25 hours
