Key Responsibilities
The Nurse Navigator manages clinical activities, coordinates patient care, and performs face-to-face visits in home settings. They also provide clinical guidance to staff, ensure documentation compliance, and participate in outreach and networking within the assigned territory.
Requirements
Candidates must hold a current RN license in Nevada and possess strong interpersonal and leadership skills. A Bachelor of Science in Nursing and previous experience in case management or leadership roles are preferred.
