Key Responsibilities

The Social Worker will provide essential social work services across various settings including acute care, maternal child care, emergency department, home health, and hospice for patients with acute, chronic, or terminal illnesses and their families. Key duties involve identifying psychosocial needs, developing strength-based care plans, managing discharge planning, providing crisis intervention, and collaborating with the multidisciplinary team.

Requirements

Candidates must possess at least a Bachelor's degree in Social Work, with a Master's degree being preferred. While a minimum of one year of experience is preferred, two years in the medical field is also preferred, and required licenses include a Licensed Bachelor of Social Work, with preferred licenses being Licensed Master Social Work or Licensed Specialist Clinical Social Worker.
