The Clinical Social Worker manages a caseload of high-risk patients by conducting psychosocial assessments and designing interventions to address social determinants of health. They collaborate with an interdisciplinary care team to coordinate services and ensure patients receive quality care to maintain independence at home.
Candidates must hold a Master's degree in social work and possess a valid LCSW or LISW license. Additionally, a valid driver's license and strong clinical and communication skills are required for this role.
