The Nurse Practitioner conducts comprehensive in-home and telehealth health risk assessments to identify chronic conditions and physical needs. They are responsible for documenting patient visits, determining diagnoses, and coordinating care with primary providers and caregivers.
Candidates must hold an active, unrestricted Nurse Practitioner license and board certification from ANCC, AANP, or NCCPA. Additionally, they must possess a valid driver's license, active BLS certification, and proficiency in electronic health records.
