The Nurse Practitioner conducts comprehensive in-home and telehealth health risk assessments to identify chronic conditions and physical needs. They provide written documentation, coordinate with primary care providers, and manage patient referrals to ensure quality care.
Candidates must hold an active, unrestricted Nurse Practitioner license and board certification from ANCC, AANP, or NCCPA. A valid driver's license, BLS certification, and proficiency in EHR systems are also required.
