Provide primary care services to high-acuity patients within their homes. Collaborate within an integrated, team-based, physician-driven model to deliver patient-centered care.
Must be a licensed Nurse Practitioner with a valid DEA license and CPR/BLS certification. Experience in geriatrics, home health, or hospice is preferred.
