Key Responsibilities
The Care Coordinator Social Worker collaborates with clinical teams to assess patient risk factors and facilitate integrated care delivery across inpatient and outpatient settings. They are responsible for comprehensive discharge planning, patient advocacy, and providing psychosocial support to patients and their families.
Requirements
Candidates must hold a Master’s degree in Social Work, Mental Health, Psychology, or Marriage and Family Therapy. A minimum of two years of direct clinical experience or a completed Master’s level internship is required.
