Position Summary

We're Hiring: Clinical Pharmacist | Ambulatory Pharmacy

Schedule: Monday–Friday, hours may vary based on clinic needs and pharmacist rotation schedules; generally between 8:00 AM and 6:00 PM

Location: Hybrid | remote work and in-clinic patient care

Practice Sites: ORMC, DPH, SLK, Health Central, and other ambulatory practice locations as needed

Preferred Requirements

Florida Consultant Pharmacist license preferred.

If not currently licensed, willingness to obtain Consultant Pharmacist licensure within the first 30 days of employment.

Position Summary

The Clinical Pharmacist will support Orlando Health's Scripts Ambulatory clinical pharmacy programs, including Enhanced Medication Education (EME) and Medication Disease Management (MDM). This role provides direct patient care through medication management, education, adherence support, chronic disease management, care coordination, and collaborative practice services.

The ideal candidate is proactive, adaptable, patient-centered, and passionate about partnering with providers to improve patient outcomes and expand innovative clinical pharmacy services across ambulatory care settings.

What You Will Do

Provide comprehensive clinical pharmacy services, including medication therapy management, monitoring, patient support, medication access and education, referrals, prior authorizations, and medication reconciliation.

• Review and verify the accuracy, appropriateness, safety, and effectiveness of prescribed medication therapy.
• Perform comprehensive and targeted medication reviews and disease-state therapy management counseling.

Collaborate with healthcare providers to identify medication-related problems, recommend pharmaceutical interventions, and optimize patient care.

Evaluate medication regimens for appropriate dose, route, administration method, therapeutic duplication, contraindications, drug interactions, cost-effective drug selection, and therapeutic outcomes.

Interview patients, monitor medication effects, make dosing adjustments under approved protocols and scope of practice, request laboratory testing, and communicate with members of the medical team as needed.

Perform medication reconciliation to identify and resolve drug-related problems, promote continuity of care and patient safety, and improve medication adherence and quality of life.

Monitor allergies, laboratory values, side effects, adverse drug reactions, therapy interactions, medication errors, adverse drug events, and patient compliance issues in accordance with pharmacy procedures.

Develop, implement, and monitor individualized pharmaceutical care plans.

Apply critical thinking and analytical skills to independently identify and resolve clinical and operational problems.

Accurately document clinical activities in appropriate systems and forms in accordance with established guidelines, standards of care, and quality improvement initiatives.

Collaborate with Pharmacy Management to develop, implement, and maintain clinical pharmacy programs in accordance with department policies, regulatory requirements, and accreditation standards.

Maintain required productivity, quality, and performance metrics.

Maintain current knowledge of clinical pharmacy practice, medications, therapies, and relevant healthcare developments.

Participate in multidisciplinary teams, quality committees, and other organizational initiatives as assigned.

Establish effective communication with patients, families, physicians, visitors, and team members, promoting a positive and patient-centered experience.

Maintain compliance with all Orlando Health policies, procedures, safety requirements, and applicable federal and state pharmacy regulations.

Maintain regular and punctual attendance consistent with Orlando Health policies and applicable federal, state, and local requirements.

Demonstrate understanding of Orlando Health's organizational structure, Human Resources policies, procedures, and guidelines.

Demonstrate strong leadership, communication, collaboration, and relationship-building skills while consistently providing high-quality care and service.

Qualifications

Pharmacy degree (Bachelor of Science or Doctor of Pharmacy) is required.

Minimum of 2 years of experience in clinical practice or specialty pharmacy, or completion of an ASHP-accredited residency.

Experience in Ambulatory Pharmacy or Specialty Pharmacy highly preferred.

Knowledge of Microsoft Office products, including Word, Excel, PowerPoint, and other graphics or presentation software.

Must be licensed by the Florida Board of Pharmacy. License must be current and unrestricted.
