Key Responsibilities
The RN Hospice Case Manager performs patient assessments and collaborates with the care team to develop and implement individualized plans of care. They are also responsible for documenting patient progress, teaching caregivers, and providing on-call services as assigned.
Requirements
Candidates must have at least one year of nursing experience and hold a current RN license in the assigned state. Additionally, a current CPR certification and a valid driver's license with reliable transportation are required.
