Key Responsibilities
The physician will provide outpatient primary care to a senior patient population with complex chronic conditions. They will manage a panel of up to 650 patients while working within a value-based care model.
Requirements
Candidates must hold an MD or DO degree and be board certified or board eligible in Family Medicine or Internal Medicine. A minimum of 2 years of outpatient experience and an active, unrestricted medical license are required.
