Key Responsibilities
Provide primary care to a panel of seniors with complex chronic conditions in an outpatient clinic setting. Collaborate with a dedicated care team to ensure continuity of care and meet quality metrics including HEDIS compliance.
Requirements
Must be a board-certified or board-eligible MD or DO in Family Medicine or Internal Medicine. Requires at least 2 years of outpatient experience and an active, unrestricted medical license.
