Key Responsibilities

The RN Case Manager oversees and coordinates discharge planning and utilization management activities for an assigned caseload in an acute care setting. They facilitate efficient patient progression through the healthcare continuum while ensuring high-quality, cost-effective care.

Requirements

Candidates must hold a current Michigan RN license and possess a BSN or an ADN with a commitment to obtain a BSN within four years. At least two years of acute care case management experience is required, with five or more years preferred.
