Key Responsibilities

The Medical Social Worker assesses patients to identify psychosocial, financial, and environmental needs, performing initial and subsequent social work evaluations during home visits. Responsibilities also include communicating findings to the team, implementing patient safety plans, and participating in the development and revision of the Plan of Care.

Requirements

Candidates must possess a Master’s Degree or Doctoral Degree in Social Work from an accredited school and have at least one year of social work experience in a healthcare setting. A current State License as a Social Worker, excellent communication skills, and reliable transportation for 50% travel are also mandatory requirements.
