Bristol Home Care and Hospice Agency is currently seeking a full-time Physical Therapist. We are a 4 star rated agency in patient satisfaction by CMS. We service the greater Bristol, CT area. We are a community non-profit agency supporting the greater Bristol, CT area. Assesses patient needs for Physical Therapy and related therapeutic services in the home care setting, develops treatment plans and goals in consultation with other treatment team members, and conducts therapy programs designed to improve functioning and enhance patients well-being.
## Schedule: M-F 8:00-4:30 with occasional weekend and holiday obligations ** All start of care responsibilities are conducted by our Nursing staff Qualifications

Requirements
Requires a minimum of a Bachelor's Degree in Physical Therapy and at least one year of relevant experience. Must hold a current Connecticut state license and a valid driver's license with reliable transportation.
