Schedule: Monday-Friday 8:30 AM-5:00 PM Weekend On-call/Holiday per need/rotation
Territory: Preble and Montgomery County and surrounding areas

Key Responsibilities
Direct Patient Care & Care Planning
Provide skilled physical therapy services in the patient's home as ordered by the attending physician.
Complete OASIS assessments and initial evaluations/admissions within 48–72 hours of referral.
Develop, evaluate, and adjust individualized Plans of Care (POC) based on patient condition and progress.
Treat patients to relieve pain, restore function, and maintain maximum performance using physical modalities (exercise, massage, heat, water, etc.).
Front-load visits for high-risk patients to help reduce re-hospitalizations.
Arrange outpatient therapy services when treatment cannot be delivered in the home.
Patient & Family Engagement
Educate patients and family members on injury/disease management, preventative measures, and safety.
Instruct patients and families on proper equipment use (wheelchairs, braces, walkers, crutches, canes, etc.).
Collaborate with the DME department and/or MSW to ensure families understand financial responsibilities for equipment.
Supervision, Documentation & Administration
Oversee and supervise therapy services provided by PTAs and Nurse Assistants; conduct supervisory visits and communicate results.
Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines:
Start of Care visits: Documented within 48 hours.
Subsequent, Discharge, & Eval/Recert visits: Documented within 24 hours.
Perform chart audits at agency sites as requested.
Participate in case conferences, orientation/training of new employees, and required in-service meetings.
Qualifications & Attributes
Licensure: Active Physical Therapist (PT) license in the state of employment.
Driver Requirements: Valid driver’s license and active auto insurance (with your name listed as a driver).
Experience:
Required: Minimum of 1 year of PT experience in an acute setting.
Preferred: Prior home care experience.
Core Competencies:
Strong clinical judgment and problem-solving skills.
Excellent communication skills to build positive relationships with patients, families, and co-workers.
Highly organized, detail-oriented, observant, and punctual.
Knowledgeable in therapy best practices and compliant with all legal rules/regulations.
Physical & Health Requirements
Health & Safety
Must pass physical position requirements and be free of conditions that restrict job performance.
TB Testing: Evidence of a current negative tuberculin skin test/x-ray required at orientation, followed by annual testing (unless prior documentation is provided).
Position involves potential exposure to bloodborne pathogens.
Physical Demands
Constant (66%–100%): Driving, computer/PDA usage, reading, writing, sitting, standing, walking, bending, reaching/stretching, speaking, seeing, and hearing (in-person and via phone).
Frequent (34%–66%): Lifting up to 50 lbs. (with or without assistance), climbing, stooping (bending at waist), and hand/finger dexterity.
Occasional (2%–33%): Distinguishing smell and taste.
