Key Responsibilities
The Advanced Practice Provider will provide full-spectrum ambulatory primary care, including chronic disease management, acute care, and wellness services for adult and pediatric patients. They will also collaborate with clinical teams to ensure high-quality patient care, documentation, and effective communication within the Patient Centered Medical Home model.
Requirements
Candidates must hold a Master's or Doctor of Nursing Practice degree or be a graduate of an NCCPA-approved physician assistant program with current Colorado licensure. A minimum of 3 years of FNP experience in an ambulatory family medicine practice is preferred, along with active DEA and CPR certifications.
