Key Responsibilities

The Discharge Nurse Navigator facilitates the hospital discharge process by conducting daily rounding and identifying barriers to a safe transition home. They also provide patient and family education regarding diagnoses, medications, and care plans to ensure continuity of care.

Requirements

Candidates must hold a Bachelor of Science degree in Nursing and possess at least two years of inpatient pediatric experience. Required certifications include CA Registered Nurse licensure, BLS, PALS, and NRP.
