Job Duties

Position Details

Facilitates entrance of new patients into care programs. Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.

  • Responsible for the coordination of pre and post hospital care.
  • Serves as the liaison between the patient and caregivers.
  • Coordinates appropriate multidisciplinary clinics.

Performs duties as a patient navigator guiding incoming calls and facilitating entrance of new patients into the multi-disciplinary program.

Facilitates entrance of new patients into care programs.

Provides patient education, promotes an understanding of the condition, and provides ongoing continuity of care for patients.

Provides clinical support to the provider and patients to assist in the diagnosis and treatment of conditions.

Collaborates with other healthcare providers in coordinating and implementing the patient's plan of care including evaluation, treatment, follow-up and surveillance.

Communicates with patient via mail, telephone, and in person regarding information pertinent to their care Coordinates patient and interdisciplinary activities to meet the needs of a defined group of patients.

  • Acts as an effective resource.
  • Coordinat's patient activities through the interdisciplinary clinics as appropriate.
  • Reinforces patient and family education based on individual needs either in person or by telephone.

Coordinates patient activities through available support services, such as support groups and social services as appropriate.

  • Maintains systems and processes to assure timely follow-up and surveillance of post-follow-up patients.
  • Provides nursing care and assists physician and procedures.
  • Educates other team members in area of expertise.

Education

Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree-Nursing (Preferred)

Requirements

Candidates must have at least two years of RN work experience and a graduate degree from a specialty nursing training program. A valid Pennsylvania Registered Nurse license and Basic Life Support certification are required.
