Job Summary and Responsibilities

As our Occupational Therapist, you will provide essential in-home occupational therapy services under physician's orders. Your role involves evaluating home safety, assessing functional needs, and implementing tailored treatment to promote independence.

Every day you will evaluate home safety, assess functional needs of physically disabled patients, then develop, plan, and implement individualized occupational therapy treatment procedures. To be successful, you will demonstrate strong assessment, treatment planning, and implementation skills for in-home care.

Excellent communication for patient education and a commitment to promoting independence and safety are essential.

Responds to requests for service and initiates referrals when appropriate. Following screening to determine need for intervention.

Assesses patients/residents to obtain and interpret data necessary for treatment planning and implementation per physician's orders.

• Interprets evaluation findings to patients/residents, family, significant others and care team.
• Develops treatment plans, including goals and methods to achieve identified goals.
• Coordinates treatment plans with patients/residents, family, significant others and care team. I
• Implements treatment directly or supervises treatment by a certified Occupational Therapy assistant and/or an Occupational Therapy aide.

• Occupational Therapist: CA, upon hire and
• American Heart Association Basic Life Support - CPR, within 30 Days
• Two (2) or more years of experience as a Registered Occupational Therapist.
