Key Responsibilities
The RN Case Manager ensures the quality and safe delivery of home health services by developing, implementing, and evaluating patient care plans. They provide ongoing assessments of physical, psychological, social, and spiritual needs while coordinating care with other health professionals.
Requirements
Candidates must be a registered nurse with current state licensure and possess at least three years of nursing experience. Proficiency in Oasis documentation and strong interpersonal and communication skills are preferred.
