The Physical Therapist will evaluate patients' physical abilities and develop individualized treatment plans to improve mobility and function in a home care setting. They will also collaborate with healthcare professionals and educate patients and caregivers on safety and self-management strategies.
Candidates must be a graduate of an accredited Physical Therapy program and hold a current Physical Therapist license in the State of Michigan. A valid driver's license, reliable transportation, and BLS certification are also required.
