Job description

Inova Schar Clinic is seeking a dedicated Sr. Clinical Pharmacist - Oncology to
join the team. This role will be full-time Monday – Friday; shift varies|
Sign-on Bonus & relocation assistance available.

Clinical Pharmacist Job Responsibilities

* Interprets, prepares, and verifies medication orders as appropriate.
* Provides clinical consultation and cost-effective alternatives.
* Issues controlled substances in compliance according to the DEA and VA Board
of Pharmacy requirements.
* Participates in the medication formulary review and approval process.
* Monitors drug therapies for interactions, allergies, and appropriateness.
* Reports adverse drug reactions promptly and accurately.
* Minimizes non-formulary procurement and supports substitutions.
* Participates in decentralized rounding with clinical teams.
* Responds to emergent bedside alerts and codes (adult and pediatric as
designated).
* Precepts learners as appropriate. 
* May perform additional duties as assigned, including, but not limited to: 

* Spearheading grant work and submission related to the practice of pharmacy.

* Lecturing and facilitating learning opportunities for pharmacy students,
PGY1 and PGY2 pharmacy residents, and practitioners.

* Performing duties on local, system. And national committees which
contribute to the development and professional advancement of pharmacy
practice.

* Serving as a leader or facilitator on a regional or national professional
organization.

Minimum Qualifications

* Education: Doctor of Pharmacy (PharmD) or Bachelor degree in Pharmacy.

* Experience: Five (5) years as a clinical Pharmacist. 

* Licensed Pharmacist in the state of VA.  

* Certification: Basic Life Support (BLS), Advanced Cardiovascular Life Support
(ACLS) for Critical Care and Emergency Medicine, and Pediatric Advanced Life
Support (PALS) within 90 days of hire. 

Preferred Qualifications

* PGY1/PGY2 Oncology residency.
* Board certified Oncology Pharmacist or certification obtained within two (2)
years of employment.

Key Responsibilities

The Sr. Clinical Pharmacist is responsible for interpreting and verifying medication orders while providing clinical consultations and monitoring drug therapies for oncology patients. They also participate in formulary reviews, decentralized rounding, and the precepting of pharmacy learners.

Requirements

Candidates must hold a PharmD or Bachelor's in Pharmacy, be licensed in Virginia, and have at least five years of clinical pharmacy experience. Required certifications include BLS, ACLS, and PALS, with a preference for PGY1/PGY2 Oncology residency and Board Certification.
