Key Responsibilities
The pharmacist will perform population health management, including chronic disease management and post-discharge follow-up for heart failure patients via virtual communication. They will also collaborate with healthcare teams to optimize medication therapy, ensure formulary adherence, and provide patient education.
Requirements
Candidates must hold a Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school and be licensed or eligible for reciprocity in the state of practice. The role requires the ability to obtain dual licensure in IL and WI within 90 days and possess strong clinical and communication skills.
