Key Responsibilities

The Nurse Navigator coordinates health care services for patients with complex conditions by developing personalized care plans and serving as an educator and advocate. They collaborate with primary care providers to monitor patient status, ensure adherence to care plans, and facilitate transitions between inpatient facilities and home care.

Requirements

Candidates must be a graduate of an accredited nursing school or hold a Bachelor of Science in Health Education with at least one year of direct patient care experience. A valid Texas Registered Nurse license and Basic Life Support (BLS) certification are required.
