Home Health Physical Therapist

Key Responsibilities

The Physical Therapist will plan and administer prescribed physical therapy treatments in patients' homes to maximize physical function, which includes assessing strengths to guide treatment goals and developing rehabilitation programs based on the physician's Plan of Treatment. This role also involves providing clear care instructions to PTAs and Home Health Aides, overseeing their work, and thoroughly documenting and reporting patient progress to all relevant parties.

Requirements

Required qualifications include a degree from an accredited Physical Therapy Program approved by the APTA, a current and unrestricted Physical Therapy license, and current CPR certification. One year of experience as a Physical Therapist is preferred, and candidates must be prepared for patient-facing duties requiring TB screening and maintaining personal vehicle liability insurance.
