Join ALL ABOUT YOU CHCS LLC as a Physical Therapist – Per Diem, Homecare in United States. Description All About You Collaborative Health Care Services, LLC is growing! We are seeking experienced per diem Physical Therapists to join our team for Stratford, Milford and the surrounding areas.

Key Responsibilities

AND RESPONSIBILITIES: The Physical Therapist provides therapeutic services in the patient’s home setting.

The services provided by the Physical Therapist will follow a specific written plan of care established in coordination with a physician.

The Physical Therapist will work to develop, implement and evaluate the plan of care.

Physical Therapist will provide coordination of the plan of care, while working collaboratively with other health professionals.

Requirements

Currently licensed in the state(s) in which practicing.

Two (2) years homecare experience, preferred.

Ability to exercise initiative and independent judgment.

Work Schedule

Per Diem position.

Day shift only - no nights.

Flexible scheduling available.
