Position Summary

We have an exciting opportunity to join our team as a Peds or Family Transplant Nurse Practitioner in Pediatric Heart Transplant in Manhattan.

In this role, the successful candidate will be responsible for the provision, coordination, and assurance of comprehensive nursing care using nursing process and pertinent standards of care, the diagnosis of illness and physical conditions, and performance of therapeutic, corrective, and prescriptive measures as appropriate for pediatric heart failure, pediatric VAD, and preoperative and post operative pediatric heart transplant patients, under direct supervision of the Pediatric Transplant Manager. Additionally, this position will be responsible for all pediatric transplant data and registry collection.

Job Responsibilities

• Performs complete health history, physical examination and psychosocial assessment of pre- and post-operative transplant patients in the outpatient clinic.
• Monitors and trends the cost effectiveness of the position, particularly as it relates to the efficiency and performance indicators established for the service.
• Assists in the preparation, implementation, and evaluation of research protocols when applicable.
• Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise.
• Supports the transplant department and the medical center in all research efforts.
• Uses evidence-based health care literature to advise and support appropriate practice changes within the designated service.
• Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement.
• Participates in educational and outreach programs for transplant patients and their families, and other health professionals to increase knowledge about transplantation.
• Serves as a clinical expert and resource for the education of peers, staff members on the transplant unit and other health professionals.
• Maintains current expertise in transplantation.
• Promotes own professional growth and development in clinical and managerial role.
• Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies. Functions as a respectful member of the health care team.
• Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of responsibilities.
• Provides consultation about the transplant process to the nursing staff responsible for the patients care in the hospital setting. Facilitates the continuity of care across the continuum of services, e. g. home care, other facilities, appointments with MDs etc.
• Participates in the development and revision of interdisciplinary and nursing standards, patient and family education materials as relevant to area of practice and expertise. Works collaboratively with the attending physician to maintain timely and appropriate communication with family members, housestaff, other transplant team members, other disciplines and ancillary services. Works collaboratively with nursing leadership on the transplant unit and elsewhere, to ensure timely communications with the nursing staff.
• Supports the mission, philosophy, standards, goals and objectives of the institution, department and service line. Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. Works collaboratively with the interdisciplinary team including the attending physician to develop and implement the therapeutic plan of care for a group of patients and ensure communication with family members.
• Acts as a professional role model to all levels of staff by incorporating expertise and related experience into all aspects of patient care. Considers needs and behaviors of specific patient age and cultural groups in all patient care.
• Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Educates patient and family to facilitate and promote healthy adjustment to the transplant process.
• For nurse practitioners, practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges.
• Prescribes/orders medical equipment, devices, physical and occupational therapy, and home health services per practice protocol.
• Coordinates discharge planning with attending physicians, inpatient nurse practitioners, social work and patient and family, to assure timely and safe discharge. Orders / prescribes discharge medications according to New York State law, including controlled substances with DEA authorization.
• Works closely with insurance case managers and patients to facilitate approval for transplantation and needed services and medications.
• Formulates a plan of care, along with the attending physician, consulting physicians, patient, family and other members of the interdisciplinary team for both preoperative and postoperative transplant patients, based on expected goals of care and length of stay, care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes.
• Takes call for organ offers, and for patient questions and problems outside of normal business hours. Coordinates all aspects of the actual acquisition of an organ for a particular patient when an organ becomes available.
• Maintains contact with assigned patient caseload between visits, based on their particular needs. Initiates appropriate response to issues and problems identified during these contacts.
• Reviews testing, differentiates between normal/abnormal findings and follows standard practice for abnormal findings, which may include further diagnostic testing or referral to consulting physicians per customary pattern of the collaborating physician.
• In collaboration with physicians, orders and coordinates laboratory, radiological and diagnostic studies for routine follow up, and as a response to problems identified in the outpatient clinic.
• All pediatric transplant data and registry including but not limited to, UNOS reportings (listing forms, TEIDI forms, Justification forms, etc), clinical registry data collection and inputting
• Any other assigned duties

Minimum Qualifications

To Qualify You Must Have

• Licensure: Current registration as Professional Nurse in New York and if appropriate, registration as Nurse Practitioner in New York;
• Education: Master's Degree in Nursing.
• Credentialed through the Credentialing Committee of the Medical Board with Delineation of Privilege approved with role specific treatment privileges;
• Certification required: CCTC after one year in role, ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical area of practice if appropriate;
• BCLS, ACLS PRI Certification/Screening is optional;
• Competence: At least five years of clinical nursing practice, ability to apply nursing process toward achievement of specific outcomes in an organized fashion, which addresses patient/family safety and satisfaction.
• Previous experience in transplantation or critical care is preferred.
• Evidence of excellent interpersonal and communication skills, creative problem solving and excellent critical thinking and leadership.
• Previously demonstrated ability to facilitate group processes and work cohesively and collaboratively as member of the interdisciplinary team.
• Effective change agent.
• Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities.

Membership in NATCO, ITNS desirable
