Full-time transplant social worker in Grand Rapids, MI. Monday-Friday, 8am-4:30pm, no weekends. MSW and LMSW required.

Key Responsibilities
The transplant social worker will collaborate with an interdisciplinary team to support pediatric transplant patients and their families with complex psychosocial needs. Responsibilities include discharge planning, crisis intervention, bereavement support, and facilitating behavioral health and guardianship concerns.
Requirements
Candidates must hold a Master's Degree in Social Work and have at least 2 years of post-graduate healthcare experience. A Licensed Master Social Worker (LMSW) credential for the State of Michigan is required upon hire.
