Key Responsibilities
The physician will manage a panel of up to 450 senior patients, providing comprehensive, value-based primary care and health coaching. They are responsible for leading a dedicated care team and collaborating across the care continuum to ensure positive clinical outcomes.
Requirements
Candidates must hold an MD or DO degree in Internal Medicine, Family Medicine, or Geriatrics, and possess or be able to obtain a Florida medical license. Board eligibility or certification in a relevant specialty and a current DEA number are also required.
