Case Manager, RN- Inpatient

Key Responsibilities

The Case Manager RN facilitates patient progression through acute care by developing and monitoring individualized discharge plans in collaboration with the interdisciplinary team. They identify and resolve clinical or psychosocial barriers to ensure safe transitions and reduce readmission risks.

Requirements

Candidates must have five years of nursing experience, including at least two years in Case Management or Emergency Medicine. Proficiency in acute care nursing principles, discharge planning, and medical necessity criteria such as InterQual is required.
