Job Description

Position: Emergency Medicine Physician, MH First

The Emergency Medicine Physician is an experienced, board-certified physician who supports MH FIRST, Monogram Health’s 24/7 virtual acute care clinic.

This physician provides real-time clinical escalation support for urgent and complex patient concerns and works closely with MH FIRST registered nurses, clinical leadership, field-based clinicians, and operations.

The ideal candidate brings strong emergency medicine judgment, communicates calmly in high-stress situations, is comfortable delivering care virtually to avoid Emergency Department utilization when appropriate.

The physician team supports 24/7 incoming patient calls, which includes overnight, weekend, and holiday coverage for patients with multiple chronic conditions. This role is primarily remote but may require presence in an office setting within home state.

Responsibilities

  • Provide real-time physician consultation and escalation support for urgent and complex patient concerns routed through the MH FIRST 24/7 triage team.
  • Conduct real-time synchronous and asynchronous virtual clinical assessments within the physician’s licensed states and determine the appropriate next step, including emergency services, urgent in-person evaluation, clinician follow-up, or continued monitoring.
  • Partner with MH FIRST registered nurses to support clinical decision-making, escalation pathways, and timely disposition of high-acuity or medically complex calls.
  • Review available medical history, symptoms, medications, care plans, and clinical data to support safe, informed, and patient-centered decisions.
  • Document virtual encounters, clinical reasoning, recommendations, and handoffs accurately and promptly in the designated electronic health record and supporting systems.
  • Coordinate with field clinicians, regional medical leadership, care management, and other clinical partners to support seamless handoffs and continuity following an acute event.
  • Participate in overnight, weekend, and holiday rotations; follow state-specific telehealth requirements, Monogram clinical standards, documentation expectations, privacy requirements, and escalation protocols.
  • Conduct video first virtual visits in all instances where it is feasible and appropriate.
  • Participate in a 24/7 physician coverage model, including assigned overnight shifts and/or equitable rotation schedules as determined by business and patient care needs.
  • Weekly admin time (2-4hrs) will be allocated for required CME and other required documentation to efficiently obtain and maintain multi-state licensure.

Position Requirements

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school and completion of an accredited Emergency Medicine residency program.
  • Current board certification in Emergency Medicine through the American Board of Emergency Medicine (ABEM) or American Osteopathic Board of Emergency Medicine (AOBEM).
  • Minimum of five (5) years of post-residency clinical experience in Emergency Medicine.
  • Active, unrestricted medical license in at least one U.S. state, with the ability and willingness to obtain and maintain the multi-state licenses and credentials required for assigned coverage.
  • Experience evaluating high-acuity, medically complex, or polychronic patients; experience with Medicare Advantage, value-based care, home-based care, telehealth, or clinical triage is preferred.
  • Comfort with virtual care, electronic health records, clinical communication technologies, and making timely decisions using incomplete or evolving information.
  • Ability to work scheduled overnight, weekend, and holiday rotations and communicate calmly, clearly, and compassionately with patients, caregivers, nurses, and interdisciplinary clinical teams.

Compensation

The base salary range for this role is $350,000 to $440,000 and reflects the range of experience and qualifications we may consider. Final compensation will be determined based on geographic location and job-related factors such as experience, skills, education, board certification, state licensure, scheduling expectations, and internal compensation considerations. While a larger range is posted, most offers will typically fall within a narrower band based on these factors.

In addition to base salary, this role may be eligible for additional benefits, including malpractice coverage, continuing medical education support, and a comprehensive benefits package.
