Key Responsibilities

The Social Worker Care Manager provides interventions and discharge planning services to facilitate smooth patient transitions across the continuum of care. They also monitor patient care plans, ensure appropriate resource utilization, and provide clinical supervision to peers and students.

Requirements

Candidates must hold a Bachelor of Social Work and possess at least one year of clinical experience. A BLS certification from the American Heart Association is required, and state licensure as a Social Worker is mandatory.
