About the Role

MEDEX STAFFING NETWORK LLC is seeking an experienced Case Manager RN for an exciting 12-week contract opportunity in Los Angeles, CA. MONDAY-FRIDAY 8am-4:30pm. Responsibilities

• Coordinate and manage patient care plans across the continuum of care to ensure optimal outcomes
• Conduct utilization reviews and collaborate with interdisciplinary care teams to facilitate appropriate level of care
• Assess patient needs, develop individualized care plans, and monitor progress throughout the care episode
• Facilitate timely discharge planning and coordinate post-acute care transitions and community resources
• Communicate effectively with physicians, payers, patients, and families regarding care coordination and discharge goals
• Document case management activities accurately in the electronic health record (EHR) system
• Advocate for patients to ensure access to appropriate healthcare services and resources

Requirements

• Active RN license in the state of California (required)
• Minimum 2+ years of case management or utilization review experience in an acute care setting
• Strong knowledge of InterQual or Milliman care guidelines preferred
• Experience with discharge planning, care coordination, and post-acute placement
• CCM (Certified Case Manager) certification is a plus
• Proficiency with electronic health record (EHR) systems
• Excellent communication, critical thinking, and organizational skills
