Key Responsibilities

The Primary Care Provider performs comprehensive in-person assessments and collaborates with an interdisciplinary team to develop and oversee participant care plans. They provide continuous medical coverage and coordinate care across various settings, including skilled nursing facilities and residences.

Requirements

Candidates must hold a certificate as a Nurse Practitioner or Physician Assistant with prescriptive authority and a valid RN state license. A minimum of two years of experience working with frail or elderly populations is required.
