Key Responsibilities

The Nurse Practitioner will manage acute and chronic conditions across the lifespan while performing preventive care and diagnostic testing. Responsibilities include conducting telehealth visits, documenting care in the electronic medical record, and collaborating with the broader care team.

Requirements

Candidates must hold a Master of Science in Nursing (MSN) and possess a current NYS Registered Professional Nurse license and CNP certification. A minimum of three years of professional nursing experience in a clinical setting is required.
