Work Schedule

Key Responsibilities
Provide personalized, coordinated healthcare to long-term care residents to improve quality of life and prevent unnecessary hospitalizations. Collaborate with a team of providers and case managers to implement a national model of care in compliance with regulations.
Requirements
Requires a valid FNP, AGNP, AHACNP, or PA license and a degree from an accredited APRN or PA program. Candidates must have 3 years of experience in clinical nursing or rehab specifically with geriatric populations.
