Key Responsibilities
The social worker will provide professional services to assist patients and families with mental health care coordination and community resource referrals. They will conduct comprehensive assessments, develop individualized care plans, and collaborate with a multidisciplinary team to manage patient transitions.
Requirements
Candidates must hold a Bachelor's degree in Social Work or a related field and possess at least one year of social work experience. A Licensed Baccalaureate Social Worker (LBSW) certification by the State of Texas is required, with a preference for a Licensed Master Social Worker (LMSW).
