Clinical Social Worker - LCSW or LICSW - Integrated Behavioral Health - Primary Care

Mayo Clinic is seeking a full-time Clinical Social Worker to join our Integrated Behavioral Health (IBH) Team. This outpatient role enhances access to behavioral health services and supports population health.

You will work as part of a multidisciplinary team, including Psychiatrists, Psychologists, APRNs, RN Care Coordinators, and Primary Care Physicians, to provide evidence-based behavioral health care and coordinate transitional care and support related to social determinants of health. In this vital role, you will:

• Conduct diagnostic assessments, determine clinical interventions, facilitate crisis intervention, and deliver short-term psychotherapy and patient/family education.
• Develop and implement treatment and transition plans.
• Refer patients to community resources to address barriers related to social determinants of health.
• Serve as an active member of an interdisciplinary team by participating in huddles and case consultation meetings.
• Contribute to Mayo Clinic's mission of research, practice, and education.
• Ensure compliance with medical and legal standards and serve as a resource for allied and administrative staff within the Section of Social Work and Department of Nursing.

This position includes specified billable patient contact hours, depending on assigned unit or site.
