Responsibilities

* Provide comprehensive outpatient Family Medicine care for patients across the lifespan.
* Diagnose, treat, and manage acute and chronic medical conditions.
* Provide preventive care, routine physical examinations, health screenings, and immunizations.
* Manage common conditions including diabetes, hypertension, hyperlipidemia, respiratory illnesses, and other chronic diseases.
* Provide age-appropriate health counseling, wellness education, and lifestyle management.
* Perform in-office procedures as appropriate to training and scope of practice.
* Coordinate specialty referrals, diagnostic testing, and follow-up care.
* Collaborate with nurse practitioners, medical assistants, behavioral health providers, and other members of the care team.
* Maintain accurate, timely, and thorough documentation in the EHR.
* Participate in quality improvement initiatives and population health programs.
* Engage in community outreach and health education programs.
* Support continuity of care and develop long-term relationships with patients and families.
* Perform other duties as assigned.
