Key Responsibilities
The provider will perform routine physical examinations and develop comprehensive medical, educational, and psychosocial care plans for geriatric patients. They will work within a multi-disciplinary team to deliver care in patients' homes and assisted living facilities.
Requirements
Candidates must hold an unrestricted State of Connecticut medical license and be a qualified APRN or PA. A minimum of two years of post-graduate experience in geriatric medicine or a related field is required.
