Key Responsibilities
Provide a full range of outcome-based medical services including patient screening, diagnosis, and treatment of acute and chronic conditions. Additionally, provide clinical guidance, mentorship, and preventive health education to patients and staff.
Requirements
Requires a Master of Social Work degree and an active state license as an LCSW. Candidates must have at least two years of full-time experience in care coordination, discharge planning, or case management in a medical setting.
