Key Responsibilities
The provider will deliver remote, patient-centered care for adults with complex medical, emotional, and social needs using a telemedicine platform. Responsibilities include developing individualized care plans, coordinating care across interdisciplinary teams, and managing chronic and acute health conditions.
Requirements
Candidates must hold an active, unrestricted license as a Nurse Practitioner, Physician Assistant, or Medical Doctor in California. A minimum of 3 years of clinical experience, preferably in primary care with geriatric or palliative populations, is required.
