Key Responsibilities
Provide comprehensive primary care to senior members within a community-based clinic model. Collaborate with a multidisciplinary team to manage patient health and address medical and social needs.
Requirements
Must be a licensed Nurse Practitioner or Physician Assistant with experience in primary care, internal medicine, geriatrics, or family medicine. Requires active BLS/ACLS certification and the ability to manage a Medicare-aged patient population.
