Key Responsibilities

The Nurse Practitioner acts as part of the clinical operations team, providing direct patient care virtually and in the home, while also training new Primary Care Providers (PCPs) on technology/EMR usage. Responsibilities include geriatric assessment, diagnosis, treatment, health education, referrals, and in-depth documentation according to quality standards.

Requirements

Candidates must function independently and collaboratively, adhering to strict departmental goals, performance standards, and regulatory compliance, practicing under a written agreement with a licensed physician. Required qualifications include a Nurse Practitioner License, Board certification, current DEA number, and BLS certification within 90 days.
