• As a core member of the Stafford Services clinical team, you will be the front-line leader driving optimal outcomes and quality improvement.
• Clinical Autonomy: Provide expert routine and acute medical care directly to SNF residents.

Comprehensive Management: Conduct thorough assessments, manage complex chronic conditions, and oversee personalized care plans.

Collaborative Leadership: Partner seamlessly with facility nursing leadership and attending physicians to ensure integrated care.

Quality Champion: Actively support quality improvement initiatives, focusing specifically on reducing preventable hospital readmissions

• Your Toolkit: Qualifications
• Active and unrestricted Nurse Practitioner License in the State of Ohio.
• Active Nurse Practitioner Certification.
• Experience Bonus: Skilled Nursing/Rehab experience is preferred.
