CareOne Home Health is locally owned and growing company seeking an RN Case Manager to cover Stanislaus County and surrounding communities. Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. Home health experience ideal, and we will provide training and support. This is a full-time position with rotating on call requirements.

Key Responsibilities
The RN Case Manager is responsible for initiating and implementing patient-specific Plans of Care and delivering quality nursing services in home settings. They must coordinate with physicians and families to evaluate patient status and manage discharge planning.
Requirements
Candidates must be a Registered Nurse with at least one year of experience and a valid license, along with current CPR/BLS certification. Reliable transportation is required, and experience with Kinnser or home health care is strongly preferred.
