Job DetailsJob Location: CareOne Home Health LLC - Monterey, CA 93940Salary Range: $55.00 - $68.00 HourlyJoin our growing team! CareOne Home Health is locally owned and growing company seeking an experienced RN Case Manager to cover Monterey County and surrounding communities. Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. Home health experience ideal, and we will provide training and support. This is a per diem position with potential for full time as we grow.

Key Responsibilities
The RN Case Manager is responsible for initiating, developing, and implementing patient Plans of Care while overseeing the delivery of quality, patient-centered nursing services. They must regularly consult with the care team, patients, and families to adjust care plans and ensure safety and positive outcomes.
Requirements
Candidates must have at least one year of experience as a Registered Nurse and hold a valid license. A current CPR/BLS certification and reliable transportation are required, with preference given to those with home health or palliative care experience.
