Key Responsibilities
The Registered Nurse Case Manager conducts comprehensive patient assessments and develops individualized care plans to meet hospice needs. They coordinate care with the interdisciplinary team, administer treatments, and provide ongoing support and education to patients and their families.
Requirements
Candidates must be a Registered Nurse with a current state license and a graduate of an NLN-accredited nursing school. A current CPR certification, a valid driver's license, and at least one year of hospice experience are required or preferred.
